This consultation questionnaire does not guarantee your booking, it is designed to give me an idea of the project overall. Once I have received the questionnaire, I will contact you by phone and we will finalize an appointment and review any questions either of us may have.
All appointments require a $100 nonrefundable deposit that goes toward the total cost of your tattoo. We accept all major credit cards.
